The procedure for transportation requests is to have the staff that is making the request complete the form, send to the building administrator to sign (type in their name) and date and then forward to transportation (sltrans@edzone.net). Lisa will email a confirmation back and you will know that your trip has been scheduled. Your staff member will need to have a roster (class list) of who will be on the bus to give to the driver when you leave for the trip, and have them be sure to complete any other details of the trip in the form.
